Targeted Health Programs
Kalwun Health Service facilitates targeted health programs to combat specific health issues in our mob across a variety of ages and health conditions.
Programs include the Deadly Choices program for schools and the community, mental health, alcohol and other drug support, school health visits, chronic disease assistance, child and maternal health support and wellbeing programs.
Care Coordination
The aim of the Care Coordination program is to provide better access to coordinated and multidisciplinary care to improve health outcomes for Aboriginal and Torres Strait Islander people who have chronic diseases.
Child and maternal health
The ‘Mums and Bubs’ child and maternal health program aims to improve birth outcomes and close the gap in rates of preterm birth, low birth weight and early infant health and wellbeing.
Connected Community Pathways
The Connected Community Pathways (CCP) program assists those experiencing palliative care or cancer.
Deadly Choices
The Deadly Choices program aims to empower Aboriginal and Torres Strait Islander people to make healthy choices for themselves and their families.
Integrated Team Care
Kalwun's Integrated Team Care program includes the Improving Indigenous Access to Mainstream Primary Care and Care Coordination and Supplementary Services programs.
School health checks
Kalwun Health Service offers a visiting service to Gold Coast schools to complete the Aboriginal and Torres Strait Islander Health Assessment to prep, primary and high school students.
Social and Emotional Wellbeing
Our team helps Aboriginal and/or Torres Strait Islander people struggling with mental health, substance use, addiction or seeking a suicide support service.
Work It Out
Work it Out is an education and exercise program for Aboriginal and Torres Strait Islander people who are at risk of having, or have been diagnosed with, a chronic disease.
