The Support at Home program has replaced the Home Care Packages Program and the Short-term Restorative Care Programme from 1 November 2025.
Support at Home can be an option if you need a coordinated approach to the delivery of your home services as your ageing journey changes.
This can include support for:
- Clinical supports, such as nursing care, occupational therapy, and podiatry
- Independence, such as help with showering, taking medications, transport, and respite care
- Everyday living, such as cleaning, gardening, shopping, or meal preparation.
It also includes opportunities to join our Wellness Centres, cultural activities and we can explore your cultural, spiritual and wellbeing goals.
Ongoing care
If you are approved to receive ongoing Support at Home services, you will be assigned one of 8 classifications with funding. This will depend on your care needs and includes funding for care management.
You can get assistance with a range of different services under Support at Home, such as:
- Domestic assistance: Help with general cleaning, shopping, laundry, and other jobs around your home.
- Home maintenance and repairs: Services to keep your home and garden safe and liveable.
- Meals for Mob
- Social support and community engagement: Stay connected with your community and culture, in person or online.
- Transport: Support to appointments, events, and everyday errands.
- Care management: Support to make, co-ordinate, and review your ongoing care plan.
- Personal care: Services if you need assistance with bathing, hygiene, dressing, mobility, and communication.
- Nursing care: Providing you clinical care e.g. wound care in the home and nursing reviews.
- Allied health and therapy: Healthcare professionals, such as podiatrists and physiotherapists.
- Therapeutic services for independent living: Acupuncture, remedial massage, and other therapies to support your health and well-being.
- Nutrition: Dietary support, supplements, and equipment for nutrition.
- Equipment and products: Walkers, shower chairs, and other equipment to help you with daily activities.
- Home adjustments: Handrails, ramps, and other changes to allow you to remain safely at home.
- Home or community general respite: Support at home, our Wellness centres to give you and your carer a short break.
Your Care Advisor will discuss with you the mix of services to meet your assessed needs and budget. You can request to change this mix at any time. You can also ask for a reassessment if your needs change.
Eligibility
Support at Home requires clients to be assessed by an assessment organisation. Our Elder Care team can support you with first steps and answer any questions you may have.
You may be eligible for subsidised aged care services if you or someone you care for is over 65, or over 50 for Aboriginal and Torres Strait Island individuals. Find out if you are eligible here.
Restorative Care Pathway
The Restorative Care Pathway is a shorter term pathway we also offer with care management. This program is designed for regaining independence and to ensure you remain safe at home following injury or illness. My Aged Care assesses and approves up to 16 weeks of intensive, personalized, short-term support—including allied health services, nursing, and other services required.

Visit www.myagedcare.gov.au to find information about aged care and accessing the services that are right for you or call the My Aged Care contact centre on 1800 200 422 from 8am – 8pm Monday to Friday and 10am – 2pm Saturday (your local time, no matter where in Australia you live).
Pricing (effective 20 April 2026)
Please note that 10% of Support at Home quarterly budgets are allocated for Care Management this includes Care Planning, coordination, reviews and monitoring of services provided and is in accordance with the Department of Health and Aged Care Support at Home Program.
