The aim of the Care Coordination program is to provide better access to coordinated and multidisciplinary care to improve health outcomes for Aboriginal and Torres Strait Islander people who have chronic diseases.

Services of the program include:

  • Provision of appropriate clinical care
  • Arranging services
  • Assisting patients to attend appointments
  • Ensuring medical records are complete and current
  • Ensuring regular reviews are undertaken by the patient’s health care team

Kalwun employs a Project Officer and Outreach Worker to provide a support and transport service to Aboriginal and Torres Strait Islander people who access mainstream primary care services.


To be eligible, clients must be Aboriginal and/or Torres Strait Islander and reside in Queensland.

Accessing the service

To access the service, clients must have a referral from their GP. Call your local Kalwun health clinic to arrange an appointment with your GP.
