Kalwun Community Care Home Care Packages provide services to help you stay at home as long as possible by providing services that support your assessed needs.

They give you choice and flexibility in the way care and support is provided and are available through Kalwun's Community Care Service.

There are four levels of Home Care Packages which are designed to give you the care you need now, but can also be changed if your needs increase later.

The types of services you can access under a home care package include, but are not limited to:

  • Personal services: assistance with personal activities such as bathing, showering, toileting, dressing and undressing, mobility and communication.
  • Nutrition, hydration, meal preparation and diet: assistance with preparing meals, including special diets for health, religious, cultural or other reasons; assistance with using eating utensils and assistance with feeding.
  • Continence management: assistance in using continence aids and appliances.
  • Mobility and dexterity: providing assistance to be assessed for and acquire and utilise aids.
  • Nursing, allied health and other clinical services: speech therapy, podiatry, occupational or physiotherapy services, hearing and vision services. Home care level 1 and 2 packages are not intended to provide comprehensive clinical or health services. Home care level 3 and 4 packages have a greater emphasis on delivering complex care in the home, including more clinical care where needed.
  • Transport and personal assistance: assistance with shopping, visiting health practitioners and attending social and wellness activities.
  • Management of skin integrity: assistance with bandages, dressings and skin emollients.

Eligibility

Home Care Packages require clients to be assessed by an Aged Care Assessment Team (ACAT).

You may be eligible for a home care package if you are:

  • an older person who needs coordinated services to help you to stay in your home
  • a younger person with a disability, dementia or other special care needs that are not met through other specialist services.

To access this service you will need to contact My Aged Care. Visit www.myagedcare.gov.au to find information about aged care and accessing the services that are right for you or call the My Aged Care contact centre on 1800 200 422 from 8am – 8pm Monday to Friday and 10am – 2pm Saturday (your local time, no matter where in Australia you live).

Please note: All fees quoted are excluding GST. Where applicable (e.g. when Kalwun is contracted to provide services to another provider), then GST will be added as per GST legislation. The above fees are for clients that have their package managed by Kalwun. Where this is not the case, a 15% surcharge will be added.

Cancellation Fee: 24 hour notice must be given for cancellation of service otherwise full fee may be charged. This can be waived at management discretion.

Exit Fee: Kalwun Community Care will charge you an exit amount to cover our administration costs in determining and making payment of unspent home care amounts. The maximum amount we will charge is $450.00.

Breakdown of costs

Your Home Care Package management is charged as per the guidelines below:

Phone Calls and Emails to clients Minimum 15 minutes: This includes leaving you a voice message and responding to an email.
Travel Time Minimum 15 minutes: Actual travel time is charged.
Research Minimum 15 minutes: This includes investigating care and support options, and may involve phone calls, Internet research or face to face discussions with expert consultants.
Communication with Service Providers Minimum 15 minutes: Service Providers can include Home Care Providers, Contractors, Doctors , Allied Health or Health Service Staff.
Client Meetings or Case Conferences Minimum 30 minutes: This can include a meeting with the client, or any internal or external meeting between staff providing a service for the client. If a client meeting is cancelled under 24 hours notice, 1 hour will be charged.
Review Preparation Minimum 1 hour
Report Writing Minimum 1 hour
